What Changed First?

Use this checklist to document changes in behavior, relationships, thinking, sleep, substances, and medication timing. The goal is to build a timeline—not to make a diagnosis.

Important: A checked item does not prove a medication caused the change. Sudden or severe changes can have many causes and may need prompt medical evaluation.

1. What changed?

Check anything that is noticeably different from the person’s usual baseline.

Connection & relationships

Mood & emotions

Thinking, suspicion & memory

Behavior & impulsivity

Sleep & physical changes

Substance use

2. What changed first?

3. Build the medication & substance timeline

Around the time things changed, was anything started, stopped, increased, decreased, switched, missed, restarted, or added?

4. Add key timeline events

Medication changes, missed doses, sleep loss, drinking, relationship shifts, major decisions, or anything else that may help establish sequence.

5. Questions worth asking

6. Safety check

Urgent evaluation may be needed for severe or dangerous changes.

If there is immediate danger, contact emergency services or a crisis service in your area. Do not abruptly stop a psychiatric medication without appropriate medical guidance unless emergency clinicians specifically direct otherwise.
